Be eligible for Supplemental Security Income (SSI) or be in one of the optional categorical coverage groups as specified in the HCS Waiver Renewal Request 1993-1998;

Be eligible for an ICF/MR I, V, VI level of care;

Have an determination of mental retardation in accordance with state law;

Have an Individual Plan of Care for waiver services which does not exceed state limits and have a choice of the HCS Program over the ICF-MR Program;
